Booking In
When the date of your procedure has been set you are required to organize for the completed consent form to be delivered to the hospital to arrange your booking. Please be advised that this consultation will take approximately 10 to 20 minutes. If you are hearing impaired, or speak another language, please arrange for someone to assist you.
This consultation confirms your doctors booking and at the same time all personal and financial details are discussed.
Please have the following articles with you when you make this call:
- Health fund name, table and membership number
- Medicare Card
- Safety Net Details*
- Pension Card*
- Health Care Card*
- Pharmaceutical Entitlement Card*
- Workers Compensation Approval Letter*
- Department of Veterans’ Affairs Number*
- Credit card details*

At this time the Front Office Manager will book you in for a pre-admission nursing assessment if applicable. If you should have any concerns regarding your hospitalisation please discuss this with the nurse.
Please advise your treating Doctor or Nurse if you are pregnant or planning pregnancy prior to admission or you have an allergies (i.e. latex).
What to Bring to hospital on admission
- X-ray and doctor’s letters
- Current medication and prescriptions*
- Physical aids (e.g. walking frame, walking sticks, crutches etc)
- Ted stockings*
- Workcover details*
- Sleepwear, dressing gown, slippers
- Personal toiletries
- Credit Card/Money to settle your account

You should not bring valuable items such as jewellery, money, gameboys, and mobile phones to hospital. The hospital is not responsible for the loss, theft or damage of valuables.
